Thank you for your interest in becoming a member of ACMHR-K. Membership is valid for one (1) year from the date of approval and is renewable annually upon payment of the prescribed membership fee.

Membership Fee: The annual membership fee is KSh 750 for your selected category. Membership becomes active upon confirmation of payment and must be renewed annually.

Payment Instructions: After submitting this form, you will receive a unique payment reference number. Use this number to make your payment via M-Pesa or bank transfer. Your membership will be activated automatically once payment is confirmed.

I hereby declare that the information provided in this application is true and accurate to the best of my knowledge. I agree to abide by the Constitution, Policies, Regulations, and Code of Conduct of the Alliance for Cardio-Metabolic Health and Research–Kenya (ACMHR-K). I understand that membership is subject to review and approval and must be renewed annually through payment of the prescribed membership fee.
